A Supported Living Service for 4 female tenants who have an Aquired Brain Injury, based in Pulborough, West Sussex.
This service aims to provide an environment that continues to enhance life and independence skills of the tenants, whilst offering an umbrella of 24 hour, social care support.
This service is a ‘step down’ from the rehabilitation model of support in recognition of those individuals who now require a good quality of life and who may not be able to manage independently in the community.
The core staff will provide support in key areas of daily living activity, a motivational approach to continued memory work and cognitive maintenance, whilst additional 1:1 support may be required to support community access.
